This Agreement is made and entered into between the client and 4-Quality Sleep Inc. (collectively referred to as "Parties") for the provision of sleep consulting services as described below:
1. Services: Sleep Consultant shall provide sleep consulting services to the Client, based on service selected. This includes but is not limited to conducting a sleep assessment, developing a customized sleep plan, providing sleep coaching and guidance, and offering ongoing support during the implementation of the sleep plan.
2. Fees and Payment: The Client agrees to remit the agreed-upon fee to 4-Quality Sleep Inc. Payment for services rendered shall be facilitated through the designated website, allowing for the flexibility of split payments. Payments can be made using all major credit cards or Visa debit cards."
3. Confidentiality: Sleep Consultant agrees to maintain the confidentiality of all information shared by the Client during the sleep consulting services. Client's personal information, sleep assessment results, and sleep plan details will be kept confidential and will not be shared with third parties without the Client's written consent, unless required by law.
4. Client Responsibilities: Client agrees to provide accurate and complete information during the sleep assessment process, adhere to the sleep plan developed by Sleep Consultant, and communicate any changes or concerns related to the sleep plan or their baby's sleep patterns. Client acknowledges that the success of the sleep consulting services depends on their active participation and commitment to implementing the recommended strategies.
5. Limitation of Liability: Sleep Consultant is not a medical professional and does not provide medical advice or diagnosis. The sleep consulting services provided by Sleep Consultant are not a substitute for professional medical advice or treatment. The client understands that the sleep consulting services are not guaranteed to resolve all sleep issues, and Sleep Consultant shall not be held liable for any outcomes or consequences related to the services provided.
6. Termination: Either Party may terminate this Agreement in writing with prior notice if there is a breach of any material terms of this Agreement. In the event of termination, the Client shall pay Sleep Consultant for the services provided up to the date of termination, this fee is half of the service package if terminated within first 5 days, otherwise full payment in required.
7. Entire Agreement: This Agreement contains the entire understanding between the Parties and supersedes all prior and contemporaneous agreements and understandings, whether oral or written, relating to the subject matter hereof.
By booking a consultation, the Parties acknowledge and agree to the terms and conditions of this Agreement.
